To Whom it may concern,
I am requesting any information on myself in order to help prove my United States Citizenship.
ANY information will be helpful (Doctor’s or nurse name, witness at birth, address at birth shot records etc; )
I was born in West Helena Arkansas, and was told that I was born at home rather than a hospital.
I have requested a birth Certificate from The Ark Dept of Health many times to no avail.
